-- ELAINE M. KIPP, 27, Chicago, at 12:49 a.m. Thursday at Racine Street and Randall Avenue, Janesville.

-- HOLIDAY DRIVE AND KENNEDY ROAD, Janesville, at 5:35 a.m. Tuesday, a sport utility vehicle driven by Julie K. Finley, 49, of 2531 Sherwood Drive, Janesville, struck Jeremiah C. Mathews, 40, of 1237 N. Lexington Drive, Janesville, who was crossing the street. Mathews was treated at Mercy Hospital, Janesville, and released.


-- AFTON ROAD AND FAWN LANE, Janesville, at 5:23 p.m. Wednesday, a vehicle driven by Brady A. Burk, 38, Beloit, collided with an oncoming car driven by Colin G. Simpson, 33, of 2025 W. Wall St., No. 10, Janesville. Simpson was treated at Mercy Hospital, Janesville, and released.


-- HIGHWAY 51 AND TERRINGTON DRIVE, Rock Township, at 12:35 a.m. Saturday, a car driven by Gary A. Walquist, 40, Beloit, slid off the road into the ditch. Walquist was ticketed for fourth-offense drunken driving, operating after revoked and driving too fast for conditions. Walquist was treated at Beloit Memorial Hospital and released.

Intoxicated driving arrests

-- JUSTIN C. ASTIN, 26, of 379 N. Tratt St., Whitewater, on Wednesday on West Main Street, Whitewater.


Accidents

-- 341 BRADLEY AVE., DELAVAN, at 8:53 p.m. Feb. 10, a vehicle driven by Michael J. Marinkovich, 33, W8589 Territorial Road, Whitewater, rear-ended a parked car, backed up and drove around the car before coming to a stop. He told police he "blacked out." Marinkovich was reportedly transported to a local hospital but his condition was not available.


-- ELKHORN ROAD AND GRANT STREET, LAKE GENEVA, at 11:42 a.m. Feb. 10, a vehicle driven by Michael H. Henry, 23, Milwaukee, rear-ended a vehicle driven by Kimberly A. Jirousek, 39, N3272 Narcissus Road, Lake Geneva, while it was stopped to make a left turn. Henry told police he didnít see the stopped vehicle. Jirousek was reportedly transported to a local hospital but his condition was not available.


-- MAIN STREET EAST OF WEST STREET, LAKE GENEVA At 5:03 p.m. Feb. 5, a vehicle driven by Camal Hibbler, 43, of 1170 Mobile St., Lake Geneva, rear-ended a vehicle driven by Brenda M. Ellis, 48, of 250 Country Club Drive, Lake Geneva, which was stopped for a pedestrian crossing the street. Hibbler told police he "stopped too quick." Both drivers were reportedly transported to a local hospital but their conditions were not available. Hibbler was cited for operating a motor vehicle while intoxicated and inattentive driving.


Charged

-- JAMIE A. TERBORG, 40, of 303 E. First Ave., Elkhorn, two counts of identity theft and one count each of forgery and misdemeanor theft. She is accused of taking checks from someone she knows and from her daughter and using them to make purchases in Elkhorn on Nov. 16 and East Troy on Nov. 8.
