-- PHYLLIS K. TURNMIRE, 44, of 321 Highland Ave., Beloit, on a bench warrant Tuesday at the Rock County Jail charging failure to appear in court on an earlier forgery charge.

-- PAUL A. MUZIK, 24, of 206 Winesap Drive, Janesville, at 2:15 a.m. today in the 2500 block of West Court Street in Janesville.

-- FINANCIAL CARD FRAUD reported Wednesday by an 82-year-old Janesville man. Loss of $4,800 from a local bank account reported.


-- BURGLARY to a residence in the 3200 block of Canterbury Lane in Janesville reported Nov. 26. Tools valued at $1,250 were reported stolen.


Accidents

-- HIGHWAY 26 AND HARMONY TOWN HALL ROAD in Harmony Township at 7:58 p.m. Sunday, a car driven by Adrianna F. Clift, 19, of 105 W. High St., Milton, turned left into the path of a car driven by Rodrigo Hernandez Lopez, 35, Watertown. Clift was ticketed for failure to yield the right of way while turning left. Hernandez Lopez was treated and released at Mercy Hospital in Janesville.


-- WEST COURT STREET WEST OF WILLARD AVENUE in Janesville at 8:04 a.m. Wednesday, pedestrian Dawn M. Rutter, 50, of 326 N. Terrace St., Janesville, walked into the path of a car driven by Marna L. LaBelle, 38, of 2117 Rockport Road, Janesville. Rutter was ticketed for pedestrian failing to yield the right of way while crossing a road. She was in good condition this morning at Mercy Hospital in Janesville.


-- HIGHWAY 51 SOUTH OF BLACK OAK DRIVE in Fulton Township at 10:56 a.m. Tuesday, a van driven by James M. Heller, 62, of 1326 E. Road One, Edgerton, slid off the slushy road and rolled over. Heller was treated and released at Edgerton Hospital.


-- MILTON AVENUE AND MORSE STREET in Janesville at 2:32 p.m. Friday, a car driven by Major Cain, 27, of 1339 Colony Court, Beloit, rear-ended a pickup truck driven by Joshua E. Jones, 24, of 333 Madison St., Janesville. Cain, who was treated and released at Mercy Hospital in Janesville, was ticketed for inattentive driving and driving while disqualified.

-- SHEILA A. JOHNSON, 38, of 3985 Creekside Drive, Janesville, with three counts of forgery in connection with the theft of more than 30 checks from a Janesville dental clinic, which were forged and cashed in a total amount of more than $42,000 from February 2005 through July 2006.


Charged

-- JAMES L. HINKLE, 45, of 220 Linn St., Janesville, with failure to report to jail Oct. 8 to serve a 30-day sentence for a ninth conviction of driving on a revoked license.


-- CHARLES S. STREULY, 44, of 221 N. Franklin St., Apt. L, Janesville, with stalking, battery, criminal damage and two counts of misdemeanor bail-jumping after a Janesville woman reported being assaulted by a man who had repeatedly harassed her.
